The sweepstakes is open to those 21 years old or older who have Type 2 diabetes or care for someone with the condition. Entrants will have the chance to win a one-on-one support session with a CDE, a gym membership and session with a local fitness expert, two tickets to a diabetes patient education conference including airfare and hotel, a meet-and-greet session with NBA Hall of Famer Dominique Wilkins (who is living with Type 2 diabetes), and more.
